Community Service

ONLY the graduating class of 2020 needs to complete the community service requirement. It is 40 hours total (5 hours per semester of attendance at CDMHS). The form is available for download below.

If you are in the graduating class of 2021-2023, you may not submit community service forms to the counseling office & your hours will not be entered. If you choose to complete community service, please keep the forms for your own records & do not turn them in to the counseling office.

The completion of 40 community service hours is required for the graduating class of 2020.
